Tuesday through Thursday; 9am to 3pm
Firearms Instruction Sheet for New Applicant
1. Complete the applications Form STS-33 and Form SP-66. Complete Part One on both Mental Health forms but do not sign them. They need to be signed in front of an Officer as 2 original documents.
2. If you are a returning applicant who has already obtained a Firearms ID Card, you will complete form 212A (yellow form) at the time of your submission. The firearms clerk will provide this form to you. Form 212A is a Criminal History Check for persons who have been issued an SBI number for firearms. You will not have to submit to electronic fingerprints at the State Approved Vendor.
3. Fees are as follows:
- $60.25 Payable directly to the State Approved Fingerprint Vendor
**Initial Firearms ID Card applications only**
- $5.00 - ID Card Payable to Medford Township
- $2.00 - Each Handgun Permit (up to 3 permits) Payable to Medford Township
- $18.00 Payable by Money Order, Cashiers Check or Certified Check
to "NJ STATE POLICE-SBI" if form "212A" (yellow form) is required.
(The ID Card and Handgun Permit fees can be paid in cash (exact amount) or personal check made payable to Medford Township.)
4. When complete and proper fees are acquired, please bring everything back plus four (4) stamped letter size #10 envelopes to the Public Safety Center between 9:00am and 3:00pm, Tuesday through Thursday. Advise the Public Safety Officer that you need to see the firearms clerk. At this time you will be given instruction for being fingerprinted at a State approved vendor and you will sign both original Mental Health forms in front of an Officer at the Public Safety Building. Once you have completed your finger printing the application process will begin.

Please note that the processing usually takes a minimum of 6 to 8 weeks.
